Graduate Certificate - Community and Health Services Navigation (CHSN)

The Community and Health Services Navigation program are a practical, diverse program that teaches students how to help people across health care, mental health, addictions, social welfare, law and justice, developmental and educational settings.

What will students learn to do in this program?

• Create transition/discharge plans 
• Conduct care team meetings to help ensure smooth, coordinated care
